Attached is the sizes of the lazy boom bag.
Since the boom is 5.850 meters long. for this reason instead of 3 circumference measurements at every 1/4 I have added 5 places G in fact now Is I -
Note the boom has a slide track the opening of the slide track slot is 1” or 12.5 mm the track begins 760 mm from the end of the boom. towards the mast.
 
Since the boom has never had a boom lazy bag, there are no eyelet sizes to refer too.
Also I took the circumference measurements with the mainsail saddled over the boom they include the boom as well as the mainsail is flopped over the boom and not on top of it, as it would in a lazy boom bag. . 
Note the mast has been re-rigged in Dyneema I have had added jack lines setup for 4 eyelet points not 3 as per your drawing, the lazy jack lines can be adjusted to the eyelets once the boom bag is installed.
 
Note: I use a cockpit cover when the boat is at anchor or on the mooring. I use a topping lift at the booms end as I do not have a solid boom vang.
Please note I would like the bag material to be in weather max.  you will have to give me a colour range.
 
Do you also make padded shroud turnbuckle protectors? if so send me a data sheet it fill in sizes and costs.
